These plain-language terms set expectations for using FORM during founder field validation and after launch.

Field-validation release

FORM is an evolving golf technique product. The implemented golf experience remains available during founder field validation. FORM may improve a feature or limit unsafe, abusive or genuinely unsupported use, but it will not invent evidence or describe an unrecorded measurement as recorded.

Who may use FORM

You must be at least 18, provide accurate account information and use an account you are authorised to control. Keep your sign-in method secure and tell us promptly if you believe the account has been compromised.

Safe use

FORM provides educational sports information, not medical diagnosis, treatment or emergency advice. Stop if movement causes pain, dizziness, unsafe contact or an unsafe practice space. Seek an appropriately qualified professional where needed. You decide whether and how to act on any suggestion.

Your recordings and instructions

You keep ownership of recordings and other content you provide. You give PeakWave a limited permission to store, transform and analyse that content only to operate the features you request, protect the service and meet the choices described in the privacy policy. You must have the right and consent to upload any content that identifies another person.

Optional learning

Using FORM does not require you to let FORM use your Check for calibration, model training, a private-media comparison study or professional review. Those are separate choices in FORM settings and can be withdrawn. A GPT comparison is a diagnostic challenger, not a source of truth, and does not change the result delivered to you. Reviewed differences become test cases and proposed rules before any later product release.

Professional references

Descriptive references to a golfer or technique family are educational comparisons, not endorsements, coaching relationships or claims that a professional uses FORM. Do not use FORM to impersonate someone or publish misleading likeness claims.

Groups and sharing

Recordings are private by default. If you create a card, invitation, group or challenge, share it responsibly. Do not harass, shame, deceive, scrape, manipulate rankings or attempt to expose another athlete's private evidence. FORM may revoke content or ranking eligibility to protect people and product integrity.

Plans and recommendations

FORM may ask for optional feedback about a possible future price. That research is not a purchase or subscription and does not change access. If FORM later offers a purchase, the price, allowance, renewal, cancellation and refund terms shown before purchase will also apply. An equipment link may produce an affiliate payment to PeakWave. Payment or an affiliate relationship never changes the canonical score for identical evidence or makes a purchase required for a drill.

Service limits

FORM aims to preserve submitted work and label uncertainty honestly, but the service may be interrupted or contain errors. To the extent permitted by applicable law, PeakWave is not responsible for indirect loss arising from reliance on a technique output. Nothing here limits rights or liability that cannot legally be limited.

Ending use

You can delete your account in FORM settings. PeakWave may restrict an account that threatens safety, privacy, security or fair participation. Data handling after closure follows the privacy policy and any legal retention obligation.
